How can we fill any lack in dentition ?

The dentition of an adult consists of 32 teeth. Although the dental surgery constantly improves and they are new methods of teeth 'rescue' there are still cases when their removal becomes necessary.

Such a situation occurs most frequently upon a tooth movement via periodontium diseases or though severe damage of hard tooth tissues through decay or mechanical trauma. When a tooth loss happens a necessity arises to fill the gap. A teeth loss not only brings a very negative cosmetic effect but also predispose basic oral cavity dysfunctions such as speaking and chewing.

In a situation of severe teeth loss a permanent damage of the temporomandibular articulation can occur. Because of the overstress and unequal load the remaining teeth are prone to fast abrasion and movement - they 'move' in free direction. All processes mentioned predispose a faster loss of the remaining teeth. The teeth replacement can be divided into temporary and permanent.

The movable ones commonly named 'prosthesis' are replacements than can be taken out the oral cavity. They are not placed permanently on a patient dentition. The permanent prosthesis cannot be taken out the oral cavity - it consists of caps, bridges and insets.

Insets are made when a cap has a severe damage and when there is no possibility to rebuild the teeth via a traditional filling. In a case of a total cap damage insets are placed in the root canal - they are named cap-root insets.

They can be done on a condition that a tooth has undergone a canal treatment. The insets can be done individually in a prosthesis laboratory for a patient or You can use a standard inset made during one visit to a dentist.

Parenthetical caps are used for rebuilding severely damaged teeth. They are placed on a previously polished tooth via special cements. Currently we use only highly cosmetic metal-ceramic caps (porcelain) or totally ceramic in the anterior compartment of dentition. In the posterior parts metal caps can be used.

Bridges are used to fill a gap of one or several teeth. A typical bridge consists of caps placed on the teeth and spans that re-create the lost teeth. The teeth that the bridge is placed on (so called 'pillar teeth') have to have a very strong mechanical durability, right placement, and be fix tightly in the bone as they transfer a chew force of several teeth. To fill the gap of a single tooth we use so-called adhesive bridges in which a cap is replaced by 'wings' glued to the bordering teeth. The usage of these bridges doesn't require teeth polishing.
Nowadays implants are used more frequently, as they are perceived as the best solution for a possible teeth loss. They can be used in the case of total teeth loss, or just a single tooth loss.

After a short healing time of 3-4 months, caps, bridges, total prosthesis can be placed on that artificial root. In a particular case we can shorter the healing process to few days. Titan because of its chemical properties is biocompatible (its acceptable by the human body and its surface binds with the bone and other soft tissues). That is why titan has found its use in medicine for over 25 years as the material for screws, rails, artificial joints and teeth roots. The success rate in the dental implants reaches 98%, which puts it not only in front of all implant procedures but also in all surgical operations. Such a prognosis puts the implants just behind your own teeth.

How does the implant treatment plan looks like ?

Basically the implant treatment schedule consists of several stages some of which are:

0. The initial examination, initial treatment
1. Planting the implant
2. Gums healing
3. Suture removal
4. Healing period of the planting area
5  Opening of the implants
6. Temporary reconstruction
7. Fitting reconstruction to the implants
8. Placement of the reconstruction.

The initial examination, initial treatment: it consists of total teeth, gums, and jaw check-up. For that we do a roentgenogram of a jaw. Based upon the outcome of the examination a doctor can perform the initial treatment procedure: removal of plaque and residue from the teeth, treatment of the mucous membrane and decay foci if necessary. In a certain cases he can widen the interproximal spaces.

Planting the implant (time: in 1 day)

This procedure is done in a totally sterile environment and a local anaesthesia. Directly after the anaesthesia a surgeon opens the gum over the bone crest in the implant placement spot. The place called the placement spot is prepared in a very special way. Only then from an absolutely sterile package an implant is taken and put into the placement spot instantly. Then the implant itself is covered by a covering screw and the gum is sutured. After the surgical procedure a wet bandage is put in mouth that has to reside there for at least 30 minutes.

Gums healing:

The inside of lips is wet so the process is very rapid. The implant area can be swelled for 2-3 days. Usage of cold compresses can eliminate the swelling completely or shorten the time of its presence.

Suture removal:

Usually after a week a doctor can remove the sutures from gums.

Healing period of the planting area (time: 3-6 weeks):

During the first week:

avoid excessive exercises, long walks, sauna sessions
clean the gums and brush the teeth; the area around the implants should be treated especially careful
a direct strong water current should be avoided on the implant area

During the next 4-6 weeks:

clean the gums and brush the teeth upon the instruction of a dentist
You can wear bridges and dentures but after each meal clean them carefully
After lighter procedure a prosthesis can be polished and covered with a soft material
Smoking tobacco should be avoided

Remember!!!

The healing process goes accordingly when there is no constant movement of either bone nor implant, so the planting area shouldn't be put upon any stress during the whole healing period.
Under any circumstances can You stress the implant area by a finger or a tongue.
The whole treatment takes 3-6 months and when a rebuilding of a bone is performed it extends up to 6-9months. Establishing the exact time is impossible due to personal patient predispositions.

Opening of the implant (time: 1 day)

When a healing is completed, implant is opened. It's done in local anaesthesia - the covering screw is removed - a special rubbery form is put to guarantee direct and strong mucous membrane connection. After this procedure You should follow several rules:

use a cold compress on a face to cool the implant area,
avoid spicy food, tobacco and alcohol,
during brushing clean the gums, rubbery form and teeth carefully, and wash the mouth after each meal

Temporarily reconstruction - after about 2 weeks a doctor removes the sutures and begins the work on making the teeth reproduction


Fitting reconstruction to the implants (time: 1 day)

The procedure during this stage consists of making a 'negative' dentition. The dental technician will use the negative to make a jaw model, which will recreate the anatomical structures in the jaw. The quality of the reconstruction depends on the precision of the model. Based on the doctor's decision the dental technician creates caps and prosthesis bridges. To fit a single cap into an implant a special device made out of pure titan is screwed. The part of a cap seen mostly porcelain is mounted onto the device with glue or a small screw.
In a meantime before the final reconstruction the temporarily one can be put.

The final stage: placing the prosthesis on the implant.
